The story continues 28 years after the Rage virus outbreak, focusing on survivors navigating a post-apocalyptic world filled with infected individuals and mutated horrors.
Survivors live on a small island connected to the mainland by a heavily defended causeway. When one of them ventures into the mainland, they uncover secrets, wonders, and horrors that have transformed both the infected and other survivors. Dr. Kelson (Ralph Fiennes) is as a force for good amidst the chaos, aiding a young boy seeking help for his dying mother in the dangerous mainland.
