American filmmaker and cyclist Bryan Fogel contacts Russian scientist Dr. Grigory Rodchenkov in order to uncover the details of a huge doping conspiracy. He discovers that 99 percent of Russian athletes are doping in order to win in their various sports, and that this has been going on since 1968.
Rodchenkov claims that Russian President Vladimir Putin knew about the doping and tells Fogel, "We are top level cheaters." Putin however, denies any knowledge of the doping scandal, even as two key players involved in it wind up dead. Fogler works to get Rodchenkov out of Russia before he meets the same fate.
